I've <br /> attached it for you. <br /> typically EACH application must have a signed MFR with it at time of submittal. One <br /> MFR can work for both applications IF the "Third Party Billing" (bottom section of the <br /> form) shows the same billing party. 99.99% of the time CONSULTANTS take on the third <br /> party billing for the field work phase. after the wells are destroyed and I sign off the <br /> permit as 'finalled', the MFR is destroyed. the third party will not receive any EHD billing <br /> for this phase after the final bill for the phase has been paid. <br /> I've attached a PDF copy of the MFR for you to complete and return to me so I can <br /> process your applics. <br /> the EHD charges a ONE HOUR fee for each well, the $125 per you submitted. And you <br /> also paid the $375 application/inspection fee with each applic. your$3625 total was <br /> correct for 23 wells on two permits. but you should know, that it typically takes more than <br /> one hour per well to destroy. the two $375 fees you paid somewhat offset any additional <br /> time per well, but experience has shown never quite to the point of leaving you with NO <br /> balance. The third party identified on the MFRs should know that there is a very high <br /> probability that after the wells are destroyed/phase is over, there will be a bill sent to <br /> cover all EHD inspection charges that exceed the $3625 you sent in. <br /> btw, I would like to have a copy of all the bore logs too. please have them on DAY 1 of <br /> the over-drills.. <br /> call if you have any questions.. <br /> mike <br /> 209.468.3454 <br /> 10/6/2011 <br />
