(Stratus) has prepared this Well Destruction Report, on behalf of <br /> Palisades Gas and Wash (PG&W), for former PG&W station No. 835 (the site), located at <br /> 2705 Country Club Boulevard, Stockton, California (see Figures 1 and 2). Stratus recently <br /> directed the destruction of 23 groundwater monitoring/remediation wells at the subject site. <br /> This work was initiated upon receiving concurrence from San Joaquin County Environmental <br /> Health Department (SJCEHD) and the California Regional Water Quality Control Board <br /> (RWQCB) that further groundwater monitoring and sampling at the site was unnecessary and <br /> that the site's environmental case could proceed towards closure. This report presents <br /> documentation of the procedures used to complete the well destruction work <br /> WELL DESTRUCTIONS <br /> Prior to initiating work activities, encroachment permits were obtained from the City of <br /> Stockton and the California Department of Transportation (CalTrans). Once the <br /> encroachment permits had been secured, Stratus obtained well destruction permits from <br /> SJCEHD. Copies of the encroachment permits and drilling permits are provided in Appendix <br /> A. <br /> i <br /> The work areas were marked for Underground Service Alert (USA) clearance 48 hours in <br /> advance, and SJCEHD, PG&W, Cal Trans, the City of Stockton, the current operator of the <br /> site service station (Tesoro), and PG&W were notified in advance of completion of work <br /> activities. The former well locations are presented in Figure 2. All work was completed <br /> under the direction of a State of California Professional Geologist. <br /> A C-57 licensed drilling contractor (All Well Abandonment, Inc., C-57 #848359) performed <br /> the well destruction activities, in the presence of a Stratus representative, between <br /> KAUSA Gasoline\0835\Reports\palisades-835-well destruction report.doc <br />
