I I <br /> 14 February 2020 AdvanCedGe o <br /> An Project No. 1]-004T rmn�� v.no ny <br /> Page 3 of 4 <br /> PROCEDURES <br /> As required, a health and safety plan will he prepared in accordance with Occupational <br /> Safety and Health Guidance Manual for Hazardous Wash Site Activities (National <br /> Institute for Occupational Safely and Health Administration, U.S. Coast Guard! and U.S. <br /> Environmental Protection Agency, 1985). Prior to drilling activities, each proposed drilling <br /> location will he clearly marked, and utility clearance obtained through Underground <br /> Service Alert. Well ahandonmentMestruction permits will be obtained from the San <br /> Joaquin County Environmental Health Department (EHD); additionally, the EHD will he <br /> notifietl a minimum of 48-hours prior to commencement of well destruction activities. <br /> Prior to well destruction activities, all wells will he checked for total depth and well integrity <br /> using a Solinsl water level meter. Any abstractions found in the wells will be removed. <br /> AGI proposes to destroy wells W4, W-7, and W-B by over-drilling. The wells will be <br /> drilled-out to the total depth of the wells utilizing a hollow stemauger drill rig equipped <br /> with 8.25-inch diameter augers. All original well construction materials will be removed <br /> (including well casing, screen, filter pack, and annular seal) by the drill rig to ensure that <br /> no residual contamination is sealed within the abandoned borehole and a vertical conduit <br /> doesn't remain. The borings will be completely backfilled to surface grade with a cement <br /> grout mixture (Portland type II) and finished at the surface with native soil or concrete to <br /> match the existing surface. The grunt mixture will consist of a ratio of not more than six <br /> gallons walerfor every g pound sack of Portland type II cement. The drill rig hollow stem <br /> augers will be utilized as a tremie pipe during deployment of the grout mixture. Any <br /> groundwater displaced by the grout mixture will be routed into properly labeled <br /> Department of Transportation (DOT)-approved 55-gallon drums. All investigation derived <br /> waste will be cenfalnerized in property labeled DOT approved 55-gallon tlrums and <br /> disposed as a non-hazardous waste. <br /> REPORTING <br /> Upon completion of Said work and disposal of investigation derived waste, a well <br /> destruction report describing well destruction activities and procedures, including <br /> documentation of waste disposal from well destruction and other activities will be <br /> prepared and submitted to the EHD and CVRWOCB. The report will be in a format <br /> acceptable b the EHD and the CVRWOCB and will be reviewed and signed by a <br /> California Professional Geologist. <br />
