10 July 2017 AdvancedGeo <br /> AGE Proposal No. 16-7172 Fnvironmental <br /> Page 4 of 5 <br /> The San Joaquin County Environmental Health Department (SJCEHD) permit will be <br /> acquired and their office will be notified for grout inspection at least 48 hours prior to <br /> conducting grouting procedures. <br /> Well Destruction Work Plan <br /> This portion of the work plan scope was prepared in accordance with Central Valley <br /> Regional Water Quality Control Board (CVRWQCB) guidelines for the investigation of the <br /> subsurface through the use of monitoring wells into the shallow groundwater and <br /> SJCEHD Standards For Well Construction and Destruction. Currently groundwater <br /> monitoring well (W-1 R) is not located near any proposed or operating discharge area and <br /> is no longer required. <br /> A total of eight (8) shallow groundwater monitoring wells (W-1 through MW-8 have been <br /> installed on site as part of the subsurface monitoring of land discharges of wastewater. <br /> Well W-1 was installed to a total depth of 20 feet below surface grade (bsg). Well <br /> construction details are known and the well integrity is good. Based on historical analytical <br /> data and the total depths of the wells, AGE proposes to fill or plug and abandon in-place <br /> the 2-inch diameter underground piping. <br /> As required, a health and safety plan will be prepared in accordance with Occupational <br /> Safety and Health Guidance Manual for Hazardous Waste Site Activities (National <br /> Institute for Occupational Safety and Health Administration, U.S. Coast Guard and U.S. <br /> Environmental Protection Agency, 1985). Prior to drilling activities, each proposed drilling <br /> location will be clearly marked and an utility clearance obtained through Underground <br /> Service Alert. <br /> Prior to destruction activities, all wells will be checked for total depth and integrity using a <br /> Solinst water level meter. Any obstructions found in the well will be removed. A neat <br /> cement Portland type 2 mixture will be installed into the well and displace the water in the <br /> wells until the casing is full. The well will be pressurized for a minimum of 10 minutes and <br /> then filled with additional Portland cement as required. The upper three feet of the well <br /> casing in the boring will be removed and then filled with additional Portland cement, <br /> surrounding the casing (2 feet wide). Remaining boreholes will be backfilled to surface <br /> grade with native, allowing the neat cement to fill and plug the horizontal surface. <br /> REPORTING <br /> The results of the investigation will be submitted in a report. The report will include a <br /> description of work performed and the results of soil sampling. Conclusions and <br /> recommendations will also be included in the reports, if applicable.
