Low Price New Used Tires Exit Interview <br /> I went over the survey form (I1-1196618) with Mr. Pawar stating a routine survey has <br /> been performed on this date(12-03-13) for Low Price New Used Tires who has been <br /> assigned this TPID # (1562878) at this site address (108 Industrial Park Dr.) Continuing <br /> I explained the TPID #is active because it generates waste tires and I informed Mr. <br /> Pawar his facility's business type is a generator and the storage method for waste tires is <br /> outdoors. I confirmed with Mr. Pawar that his facility is a new and used tire dealer. I <br /> informed Mr. Pawar that Low Price New Used Tires is in compliance, no violations or <br /> areas of concerns are noted. <br /> I then informed Mr. Pawar a comments page would be included with the Low Price New <br /> Used Tires. I informed Mr. Pawar again no violations and areas of concerns were noted <br /> then I went over the notes I wrote on my cominents page. <br /> At the conclusion of the interview with Mr. Pawar I asked if there were any questions or <br /> concerns he had regarding the waste tire survey, he replied no, so I requested a signature <br /> to complete the survey. Mr. Pawar signed the survey form and I provided him a copy of <br /> the page 1 survey form and comments pages for his records. <br /> Tires 4 Less Exit Interview <br /> I went over survey form I1-1196619 with Mr. Pawar stating a routine survey has been <br /> performed on this date (12-03-13) for Tires 4 Less who has been assigned this (TPID <br /> #1661994) at this site address (260 Button Ave.) Continuing I explained the TPID#is <br /> active because it generates waste tires. I informed Mr. Pawar his facility's business type <br /> is a generator and hauler. Onsite the storage method for waste tires is indoors. I <br /> confirmed with Mr. Pawar that his facility is a hauler and, a new and used tire dealer. <br /> I then read to Mr. Pawar that Low Price New Used Tires is in violation for not submitting <br /> CTLs within 90 days to Calrecycle and for not complying with manifest system <br /> requirements as a registered waste tire hauler. Mr. Pawar became upset and began stating <br /> reasons why the violations shouldn't be noted on the report. <br /> I continued with the interview going over my findings noted on the comments forms with <br /> Mr. Pawar. While reading my findings for the violations, Mr. Pawar, stated several times <br /> I understand, in attempt to conclude the waste tire survey early. I requested a signature, <br /> from Mr. Pawar and asked if there were any questions or concerns. <br /> I still had the survey form still in my hand; Mr. Pawar grabbed the inspection report and <br /> tore it from my hand and, stated he plans on writing his own statement on the report. I <br /> told Mr. Pawar not to write on the inspection report. Mr. Pawar then states he won't sign <br /> it so, I stated that is fine that I would write on the report, refused to sign and provide a <br /> copy to Mr. Pawar. At that moment Mr. Pawar, made an action to write on my report. I <br /> attempt to take back the report by gripping the top half of the survey form. Mr. Pawar <br /> 2 <br />
