(Stantec) on behalf of 7-Eleven, Inc., <br /> to document the destruction of groundwater z monitoring wells associated with the environmental <br /> activities at the 7-Eleven Store #20632, located.at 4627 DaVinci Drive, in Stockton, California <br /> (Figures 1 and 2).This work was completed in accordance with Stantec's Work Plan for Well . <br /> Destruction (Work Plan), dated June 1.9, 2013.This Work Plan was approved in a letter from the San <br /> Joaquin County Environmental Health Department (SJCEHD) dated June 24, 2013. A 60-day public <br /> participation period was required prior to the commencement of well destruction activities; in a <br /> letter dated February 14, 2014, the SJCEHD requested that 7-Eleven proceed with the well <br /> destructions. Copies of regulatory correspondence are included in Attachment A. <br /> Permitting/Utility Clearance <br /> Stantec obtained well destruction permits from the SJCEHD (Attachment B). Prior to conducting <br /> subsurface work at the site, Underground Service Alert was contacted to delineate subsurface <br /> utilities near the site with surface markings. In addition, a private utility locator service was used to <br /> delineate subsurface utilities at the site.The to five feet of all the wells with the exception of <br /> p ( p <br /> MW-2, MW-4R, and SPV-2) were cleared with an air knife and vacuum rig to ensure that no <br /> unknown underground utilities were present. <br /> Health and Safety <br /> Stantec modified our existing site-specific Health and Safety Plan for the scope of work as required <br /> by the Occupational Health and Safety Administration Standard "Hazardous Waste Operations <br /> and Emergency Response"guidelines (29 CFR 1910.120).The document was reviewed and signed . <br /> .by Stantec personnel and subcontractors prior to performing work at the site. <br /> Design with community in mind <br />
