i <br /> May 21,2014 <br /> Page 2 of 3 <br /> Reference: Well Destruction Report <br /> 7-Eleven Store #20632 <br /> 4627 DaVinci Drive <br /> Stockton; California <br /> Well Destruction <br /> On March 24 through 28, 2014, Stantec supervised the destruction of seven groundwater_ <br /> monitoring wells(MW-1, MW-2,MW-3, MW-4R, MW-5, MW-6, and MW-7) and two remediation wells <br /> (SPV-1 and SPV-2), by National Exploration, Wells, and Pumps (National EWP) of.Richmond, <br /> California (Figure 2 and Table 1). <br /> Wells MW-1, MW-3, MW-5, MW-6, MW-7, and SPV-1 were destroyed by overdrilling each well with <br /> hollow-stem augers to completely remove all well materials.Each void was then backfilled from <br /> total depth to the surface with neat cement grout. <br /> Due to the close proximity of underground utilities, wells MW-2 and-SPV-2 were destroyed by <br /> pressure grouting,which consisted of filling the well and filter pack with neat cement grout <br /> through a tremie pipe from total depth to top of the well casing.The grout was allowed to settle <br /> and then topped off before pressure was applied to the wells at 25 pounds per square inch for ten <br /> minutes. The top of these well locations was finished with asphalt. <br /> Due to the presence of a large amount of:pea gravel around MW-4R, it was not possible to clear <br /> the hole to five feet using an air knife and vacuum truck.The SJCEHD approved destruction of this <br /> well by pulling the casing out while filling the void with neat cement grout from the bottom up <br /> through the casing.The end cap of the casing was knocked out with a steel.rod and left in place. <br /> SJCEHD approval of the destruction methods for MW-2, MW-4R, and SPV-2 are included in <br /> Attachment A. <br /> Following destruction, all well boxes and concrete aprons were removed. <br /> Copies of the field notes summarizing well destruction activities are provided,in Attachment.C. <br /> Copies of the California Department of Water Resources (DWR) Well Completion Reports are <br /> provided in Attachment D. <br /> Waste Disposal <br /> Soil and debris generated during well destruction.activities was temporarily stored onsite in a roll- <br /> off bin.The bin was picked up by American Integrated Services, Inc. on April 7, 2014 and <br /> transported to Potrero Hills Landfill, in Suisun, California. Decontamination water was temporarily <br /> stored in a 55-gallon drum; this drum was picked up by Belshire Environmental Services;Inc.on <br /> April 15, 2014, and transported by Nieto &Sons Trucking, Inc. to Demenno Kerdoon in Compton, j <br /> California on April 24, 2014.
