1600 West Durham Ferry Road <br /> Page 2 <br /> PHS/EHD in correspondence dated February 23, 1996 indicated that once additional groundwater monitoring <br /> and sampling information was available, permit applications,encroachment agreements, permit fees ($89 per <br /> permit) and a site map should be submitted to implement the "Work Plan: Additional Site Characterization <br /> dated January 29, 1996 that proposed to install three additional shallow monitoring wells and one deep <br /> monitoring well. PHS/EHD also requested that an addendum to the work plan be submitted with the next <br /> quarterly report to explain how the vertical extent of soil contamination will be determined. Compliance, with <br /> the schedule to complete the additional investigation proposed in Geological Technics correspondence dated <br /> February 28, 1996, has not been demonstrated. <br /> Please note that Table 2. Summary of Analytical Data - Groundwater that was submitted included errors <br /> regarding the sampling date. Please make the corrections and submit it immediately. <br /> In summary, your compliance with PHS/EHD directives is in jeopardy. The results of the initial investigation, <br /> Apra 5; 1995, provided information regarding the extent of soil and groundwater contaminatign to the east <br /> and south of the site. While there could be further definitive information which may be obtained by the <br /> exploratory soil gas survey proposed, PHS/EHD believes that the monitoring well locations proposed in the <br /> January 29, 1996 Work Plan would provide valuable information regarding the contaminant plume's boundaries. <br /> In order to remain in compliance with PHS/EHD's directives,you must proceed with implementation of the <br /> January 29, 1996 Work Plan. The San Joaquin County Public Works Department and the off site property <br /> owner's permission to drill should be requested by January 20, 1997 and copies of the requests submitted to <br /> PHS/EHD. Once access agreements have been granted, they should be submitted with PHS/EHD's permit <br /> applications and fees.
