Spill Prevention, Control and Countermeasure Plan <br /> Tesla Treatment Facility Page 20 of 45 <br /> 2.4.6 State Reportable Spills <br /> Each owner or operator of a facility shall, upon discovery, notify Cal-OES and SJCEHD of the <br /> discovery of a spill or discharge of one barrel (42 gallons) or more of oil in or on any waters of the <br /> state, or discharged or deposited where it is, or will be discharged in any waters of the state <br /> [Subdivision (a) of Section 13272 of the California Water Code]. <br /> 2.4.7 Spill Notification <br /> A telephone report to Cal-OES and SJCEHD is required after a discharge equal or greater than the <br /> federal reportable quantity or state reportable quantity. Hazelton Avenue <br /> Stockton, California 95205 <br /> Phone: 209-468-3420 <br /> The following information about the spill incident would be helpful to have when making a <br /> telephone report: <br /> ® Name, location and telephone number of caller. <br /> ® Location of the discharge incident. <br /> ® Date and time of the discharge. <br /> ® The type of material discharged. <br /> The source of the discharge (truck, tank,piping, etc.). <br /> ® Description of all media affected by the discharge. <br /> ® Estimate of the total quantity discharged. <br /> ® Any damages or injuries caused by the discharge. <br /> ® Actions anticipated or in progress to stop, contain, remove and mitigate the effects of <br /> the discharge. <br /> ® Need for an evacuation or medical response. <br /> ® Names of individuals and/or organizations that have been contacted. <br /> ® Type of outside assistance required, if any. <br />
