Environmental Health PERWISERVICES <br /> P.O.Box 2009 <br /> Stockton,CA. 95201 <br /> Dear Eric: <br /> Ultramar Project Engineer, Mr. Paul Clark, has asked us me to furnish any <br /> and all information you have requested pertaining to their project,Beacon Oil Station#695 <br /> located at 900 Cherokee Lane Lodi Ca. <br /> In reference to your memo dated 11-24-93 regarding the applied for closure <br /> permit: <br /> Item# 1 Attached is a site plan showing all installed monitoring well locations to date. <br /> Extreme caution will be taken by us as contractor to preserve all indicated wells,if <br /> inadvertently any are damaged you will be notified immediately and all work that would <br /> jeopardize closure or repair of the affected well will be ceased until you can respond. Prior to <br /> starting any excavation USA underground alert will be notified of our proposed activities and <br /> all areas will be marked out as required in white paint. (see attachment#-1) <br /> Item#2 Attached is our site health & safety plan as requested this plan is used for <br /> both tank removal& new construction. (see attachment#-2 <br /> Item#3 Attached are completed pages of your closure permit application#3,#4,#5, <br /> and # 10. (see attachment#-3) <br /> Item#4 Within our site health & safety plan on the second page is an area map with <br /> the most direct routing to the nearest hospital. (see attachment#-3) <br /> In reference to your memo dated 12-01-93 regarding the application for <br /> installation permit of new tanks and lines: <br /> Item# 1 Attached is our drawing#T-1Ga please insert it in all sets of plans submitted <br /> behind sheet# T-1G,we hope it addresses your requirements. (see attachment#-4) <br /> Item # 2 There are no plans to install ball float valves into the phase two vapor system. <br /> We are installing new 95% shut off OPW 61-SO-C410 drop tubes into all tanks which should <br /> eliminate the ball float requirements. <br /> Item #3 Prior to operation of the station, but after completion of all surface <br /> restoration Ultramar will be performing a final precision tank test. Additionally we will be <br /> maintain a 3 to 5 lb. static pressure on all secondary lines, and a 20 to 30 lb. static pressure <br /> on all primary product lines until the surface areas are closed as referred to in the scope <br /> of work item # 24. <br /> P.O.
