CONTINUATION FORM <br /> OFFICIAL INSPECTION REPORT <br /> Facility Address: 1550 Shaw Road, #C, Stockton Page 3 of 5 <br /> Program: Hazardous Waste <br /> Date of Inspection: 09/21/2009 <br /> Date of Final Inspection Report: 09/28/2009 <br /> SUMMARY OF VIOLATIONS <br /> Class II: #11, 12, 19, 20, 21, 27, 82, 83, 89 <br /> #11 66262.12(a) — (Repeat Violation) EPA number is inactive at time of this <br /> inspection. A generator shall have an active EPA number to store, store, <br /> and dispose Hazardous Waste (HW). Obtain an active EPA number <br /> immediately,. <br /> #12 66265.31 CCR— Floor and equipment are covered with contaminants from <br /> the following potential sources: <br /> - Sulfuric Acid Strip Bath <br /> - Chrome Strip Bath <br /> - Copper Acid Plating Bath <br /> - Nickel Plating Bath and Nickel Rinse <br /> - Chromic Acid Plating Bath and Chrome Rinse <br /> - Aluminum Rinse Bath <br /> - Electro Cleaner <br /> - Two Dry Sandblast units <br /> - Various open containers (See Item #21) <br /> - Cyanide Copper Plating Bath, Cyanide Copper Filter Vat, and other <br /> cyanide contaminated wastes. This is a Serious hazard due <br /> to potential formation of cyanide gas from <br /> contamination with any acid. <br /> Facilities shall be maintained to minimize the possibility of release of HW <br /> which could threaten human health or the environment. Immediately, <br /> have appropriately trained personnel clean up, store, and dispose of all <br /> HW on site. <br /> #19 25201(a) HSC — Stored HW in the following containers greater than 90 <br /> days: <br /> - Three Black Poly 55-Gallon Drums: <br /> -- Nitric Rinse dated (accumulation start) August 22, 2006 <br /> -- Nickel Rinse dated August 23, 2006 <br /> - Nickel/Copper Sludge dated July 25, 2006 <br /> - Three Blue Poly 55-Gallon Drums: <br /> Powder Dust Floor Sweep dated August 13, 2008 <br /> - Zincates Bath Sludge August 13, 2008 <br /> - Nickel / Nickel Copper Sludge dated August 13, 2008 <br /> - One 5-gallon Black container of"Zincates Spent' with no start date <br />
