The above <br /> referenced site was placed in the PHS/EHD Local Oversight Program (LOP) in <br /> December 1989. <br /> PHS/EHD has completed review of the report prepared by Wm. J. Hunter & Associates <br /> dated August, 1994 titled Site Assessment Report and Request For Site Closure. <br /> The report contains site assessment information that indicates on and off-site <br /> soil contamination due to a release from the underground storage tank previously <br /> used at your site. <br /> PHS/EHD Technical Review Committee (TRC) met August 24, 1994 to review your site <br /> information and request for closure. Soil analytical results from the last phase <br /> of investigation performed July 20, 1994 evidenced residual soil contamination <br /> remains at unacceptable levels as was documented earlier in the investigation. <br /> PHS/EHD TRC recommends a groundwater investigation be conducted to determine <br /> groundwater conditions and gradient.
