27 January 2016 RECEIVEDFEB 0 1 2016 <br /> AGE Project No. 14-3289 <br /> Page 2 of 2 <br /> ENVIRONMENTAL <br /> HEALTH DEPARTMENT <br /> by over-drilling the wells to their total depths of 5 feet bsg utilizing a 4.0-inch diameter <br /> hand auger. All well construction materials were removed from the wells during <br /> destruction. <br /> Following over-drilling, each well was grouted to near surface using a Portland cement <br /> grout mixture, well boxes were removed and the surfaces at each location were <br /> completed with a concrete patch. <br /> Over the course of the over-drilling and well destructions, approximately 600 pounds or <br /> one 55-gallon drum containing waste derived from the well destructions, were removed <br /> by Instrat Inc. of Rio Vista, California.
