containment repair, which was conducted at the end of 2002 (Attachment 12). <br />On July 2, 2004, Ms. Le received permits from Donlee Pump for sensors that were replaced on September <br />21, 2001 and June 30, 2003 (Attachment 13, 14.) <br />On July 8, 2004, Ms. Le received a copy of the facilities monitoring records from January 1, 2003 -July 7, <br />2004 (Attachment 15). None of the monitoring records received indicate alarms or any problems had <br />occurred during the recording period. None of the monitoring records indicated alarms or any problems <br />even for days that maintenance records were found. <br />On August 6, 2004, Ms. Le witnessed the testing and passing of spill buckets during an EVR upgrade <br />at the facility. <br />On June 13, 2005, Ms. Le sent Village West Marina a letter stating that the annual testing needed to be <br />performed immediately (Attachment 16). <br />On June 29, 2005, Ms. Le performed a facility inspection for the end of the fiscal year (Attachment 17). <br />Alarms had occurred in February and March and the facility did not have a maintenance record for <br />them. Ms. Le requested an explanation for the alarms and subsequent correction. <br />On July 8, 2005, Ms. Le witnessed the monitoring certification and performed an inspection <br />(Attachment 18). A line leak detector failed at the time of inspection but was replaced at the time of <br />inspection and a permit was obtained as required by EHD (Attachment 19.) <br />On July 11, 2005, Ms. Le received results from the July 8, 2005 testing (Attachment 20.)
