This inspection report supercedes the March 13, <br />2014 inspection report. The report has been updated to include the findings from today's inspection. <br />Class 11 violations: (201, 204, 302, 305, 308, 311) <br />201. Detailed alarm logs were not found on site. The following alarm records were observed in the <br />facility daily tank log: <br />8/4/13 "Fuel alarm went off twice" <br />8/11/13 "Fuel alarm annular went off twice" <br />8/14/13 "Premium annular alarm" <br />8/21/13 "Annular space alarm went off 7 times" <br />8/22/13 "Annular space alarm" <br />8/23/13 "Annular space alarm" <br />9/6/13 "Annular space alarm keeps going off' <br />9/7/13 " " " " " <br />9/7/13 (second entry for this date) <br />9/9/13 "Annular/fuel alarm" <br />9/10/13 "Fuel alarm and short alarm" <br />9/10/13 (second entry for this date) "Cl, C2, L2" <br />The records do not indicate which sensor(s) are in alarm or the type of alarm. These records shall be <br />kept on site for at least 3 years. Immediately locate and maintain the alarm logs on site and submit <br />copies to the EHD by April 13, 2014. REPEAT VIOLATION <br />204. The designated operator failed to attach complete alarm histories to the monthly inspection <br />reports. The alarm printouts do not include the annular sensors (Cl and C2). The designated operator <br />failed to document all the alarms from the alarm history on the September 2013 designated operator <br />monthly inspection report and failed to check that they were responded to appropriately.
