No dates were listed. The attached work order was for September 10, 2013 and noted C1, C2, and L2 alarms. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. REPEAT VIOLATION 302 & 308. The premium STP sump was observed with the test boot not pulled back and the Schrader valve pointed up. Any potential release from the primary containment into the secondary containment in the premium piping does not flow into a collection sump. All secondary containment systems shall be sloped so that all releases will flow to a collection sump located at the low point of the underground piping. The test boot was adjusted by the technician at the time of inspection to ensure the valve is downturned. REPEAT VIOLATION 305. The Bravo box float and chain in the UDC 2 and UDC 3 sumps failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the floats and verified functionality. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
