The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak <br />detector, retested it, and it passed. <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by April 13, 2014, <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation, along with any supporting paperwork. <br />Note: Maintenance records on site indicate several inspections were conducted by technicians to <br />troubleshoot the premium annular sensor, but no maintenance records were available for repairs to the <br />91 annular sensor. On September 10, 2013, the maintenance record stated that the problem was traced <br />to bad underground wiring and additional work would be required. No maintenance records were found <br />after that date. No additional alarms were documented after that date. According to the technician on <br />site, Luis Torres, and the facility contact, Chris Phelps, the technician most likely called to check on the <br />status of the sensor, was told it was no longer in alarm, and did not follow up with the planned repairs. <br />Provided: Return to Compliance Certification form <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT MY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Title: <br />Stacy Rivera (209) 468-3440 <br />7!m-�i�vedB <br />EHD 23-02-003 Rev 04119/12 CONTINUATION FORM <br />
