El Dorado St. <br /> Stockton, CA <br /> Project No 723 2 <br /> Apn123, 2001 <br /> 1.0 INTRODUCTION <br /> On August 14, 2000, Mr Jeff Wong of SJC PHS/EHD issued a letter directing that a work <br /> plan for the abandonment of the domestic well at 60 E Castle Street be submitted In our <br /> October 13, 2000, "Domestic Well Abandonment" Work Plan we proposed a two phased <br /> approach to the destruction of the well In phase one, the well was to be investigated by <br /> submersible camera to determine its condition In phase two, the well would be destroyed <br /> in accordance with the appropriate regulatory requirements <br /> 2.0 SCOPE OF WORK <br /> 2.1 Well Video Survey <br /> On January 16, 2001, GTT staff mobilized to the site to supervise the videotaping of the <br /> well Mr William Hennch of NORCAL Geophysical Consultants Inc conducted the video <br /> of the well and Mr Jeff Wong of the SJC PHS/EHD was on site to witness the work <br /> The following information was gleaned from the video <br /> • The casing consisted of pipe sections with welded/riveted scams <br /> • Light scale was present from surface to depth, with heavy encrustation at depths of 32, <br /> 37 and 39 feet below grade surface (bgs) <br /> • At depths greater than 50 feet, heavy encrustation was present <br /> • At 69 feet bgs, an obstruction was present that appeared to be cut off wires surrounded <br /> by silt <br /> Mr Wong, upon considenng the evidence presented in the video, directed that the well be <br /> • destroyed by blast perforation <br />
