i <br /> Site Health and Safety Plan <br /> California Tank Lines Facility - <br /> Sierra-Pacific Project No. 96-620.01 <br /> 13 .0 MEDICAL SURVEILLANCE PROGRAM <br /> The purpose of the medical surveillance program is to assess the health status of personnel <br /> prior to work; to monitor personnel for evidence of post-project adverse health effects; and to <br /> determine their suitability for future work assignments of this type. All personnel who will be <br /> working on this project must undergo or be participating in annual medical evaluations before <br /> participating in these operations. Baseline and periodic health assessments for project <br /> personnel shall be consistent with OSHA regulations prescribed in 29 CFR 1910, especially <br /> those in 1910. 120 - Hazardous Waste Operations and Emergency Response. <br /> At a minimum, the examination should include : <br /> • Medical and organization histories ; <br /> • Physical examination; <br /> 0 Vision test; <br /> • Urinalysis; <br /> • Blood chemistry panel and complete blood count; <br /> • Pulmonary function tests and respirator use clearance ; and <br /> • Optional tests (need to be determined by the examining physician) include X-rays, <br /> electrocardiograms, and bioassays. <br /> If the SSO determines that significant exposure to hazardous materials has occurred in these <br /> operations, a follow-up medical exam shall be conducted on the exposed individuals. If an <br /> individual has completed an occupational medical exam within the past 12 months, the <br /> examining physician may determine that another complete exam is unnecessary. <br /> Sierra-Pacific Groundwater Consultants, Inc. 30 <br />
