If yes: which sensors initiate
positive shut -down? (Check all that apply) ❑ Sump/Trench Sensors; ❑ Dispenser Containment Sensors.
Did you confirm positive shut -down due to leaks and sensor failure/disconnection? ❑ Yes; ❑ No.
❑ Yes
❑ No*
For tank systems that utilize the monitoring system as the primary tank overfill warning device (i.e. no
® N/A
mechanical overfill prevention valve is installed), is the overfill warning alarm visible and audible at the tank
fill point(s) and operating properly? If so, at what percent of tank capacity does the alarm trigger?
❑ Yes*
® No
Was any monitoring equipment replaced? If yes, identify specific sensors, probes, or other equipment
replaced and list the manufacturer name and model for all replacement parts in Section E, below.
® Yes*
❑ No
Was liquid found inside any secondary containmentstems designed as
sy gn dry systems? (Check all that apply)
❑ Product; ® Water. If yes, describe causes in Section E, below.
® Yes
❑ No*
Was monitoring stem set-up reviewed to ensure proper settings? Attach set up Eports, if applicable
❑ Yes
® No*
Is all monitoring ui mento erational per manufacturer'sspecifications?
X in Section E below, describe how and when these deficiencies were or will be corrected,
E. Comments: The Veeder-Root was basically inoperable. The display cycled between all the sensors indicating
that they were unreliable and nothing changed when the sensors were put into alarm. Sensor #S was raised
about 1" but that was because of the length of the wire and the way it was looped over the piping. There was no
liquid in the sump that would indicate that it was done to avoid an alarm condition, and the sensor did not work
anyway. There was no liquid in the dispenser pans but there was about a foot of water (probably from the
sprinkler system on the lawn) in the Gasoline piping sump and the annular probes were nowhere near the
bottom of the annular space. When the operator of the facility learned of the conition of the monitoring system
they voluntarily shut down the fueling operation and called Elite IV to do the repairs.
Page 2 of 3 03101
