EI Dorado St. RO#: 035 <br /> Stockton , CA, 95210 <br /> On September 20 , 2004 , San Joaquin County Environmental Health Department <br /> (EHD ) conducted a field inspection at the above referenced site during a <br /> quarterly ground water monitoring and sampling event and noted significant <br /> deficiencies that will require your immediate attention . <br /> Most of the monitoring , vapor, and sparge wells associated with this site have <br /> wellhead concerns that provide a contaminant migration hazard to the shallow <br /> ground water beneath the site . Nearly all traffic box lids are unsecured . <br /> Bolts/screws that typically secure the traffic rated steel lids in place are missing <br /> or broken . <br /> Additionally, the required water-tight locking well casing caps were in such <br /> disrepair that surface liquids were being allowed to enter the casing , stain the <br /> inside PVC wall , and eventually reach the ground water. All caps that did have <br /> locks on them were so loose that the technicians at the event were able to just <br /> pull on the lock and the well cap just came off, with the lock still closed . <br /> Some of the large concrete pedestals/pads that surround the traffic boxes are <br /> heavily damaged , with large cracks that have allowed the pieces to separate and <br /> settle to different levels . The traffic boxes surrounded by these broken pads are <br /> unsecured , leaning , and not are not even providing minimal protection to the <br /> wellhead . Many wells have had the grout seal either removed or lowered to <br /> accommodate vapor lines or larger traffic boxes and , as a result , the wellheads <br /> are not sealed and liquids that enter the traffic box can potentially migrate down <br /> the casing and the borehole . <br /> These conditions are a violation of the San Joaquin County Environmental Health <br /> Department (EHD ) Well Standards and are subject to penalties unless repaired <br /> immediately. <br />
