Mr Michael Infurna,PHSIEHD December 3, 2003 <br /> Well Destruction Letter <br /> ARCO Station 2130 <br /> Page 2 <br /> Well Destruction <br /> Stratus retained Woodward Drilling Company of Rio Vista, California (C-57 #710079), to <br /> destroy well MW-10 on September 24 and 25, 2003 Following removal of the asphalt <br /> surface covering the well, the dnlling contractor utilized hand tools to remove the well <br /> cover and additional soil and gravel material beneath the roadway surface Well MW-10 <br /> was destroyed by overdnlling to approximately 75 feet bgs using 8-inch diameter hollow <br /> stem augers A "casing chaser" dnll bit was attached to the lead hollow stem auger to <br /> ensure that the string of augers stayed within the original well boring Once all of the well <br /> materials and the drill bit were removed, the borehole was backfilled with neat cement <br /> Fast setting ready-mix concrete was placed in the upper 3 feet of the borehole Once the <br /> ready-mix concrete had sufficiently cured, an asphalt patch was placed on top of the <br /> borehole flush with the surrounding roadway surface <br /> Stratus submitted a well destruction form to the California Department of Water Resources <br /> (DWR) to document the destruction of well MW-10 A copy of the DWR form is provided <br /> in Appendix B <br /> Waste Management <br /> Drill cuttings and wastewater generated were placed in properly labeled, DOT-approved, <br /> 55-gallon steel drums and transported back to the site pending disposal Dillard Trucking <br /> Company of Byron, California, transported the drill cuttings and wastewater to an <br /> appropriate facility for disposal in October 2003 <br /> Roadway Re-Surfacing <br /> The encroachment permit issued by CSPWD required that Hammer Lane be re-surfaced in <br /> the immediate vicinity of the abandoned well Dollar's Construction, Inc of Manteca, <br /> California (contractor's license #560807) completed the re-surfacing activities on <br /> November 18, 2003 An area (approximately 20 feet by 12 feet) was ground-down to a <br /> depth of approximately 15 to 3 inches below grade Hot asphalt was placed within the <br /> ground-down area and compacted with a mechanical roller The work was performed to <br /> meet the specifications outlined in the encroachment permit <br /> 1. <br /> P 1Bp-Arco for URS1213o1Correspondence12130_MW-10 well destruction doc 57TAW5 <br />
