A"No Further Action"letter for this site will not be issued until the monitoring wells <br /> are destroyed. These wells have not been destroyed to date. <br /> You had previously indicated to EHD of your desire to keep the two wells after closure. As a part of the <br /> condition of the"Underground Storage Tank Closure Permit"approved by EHD on December 10,1998,you <br /> were required to submit to EHD a written request to keep the monitoring wells. When this request was not <br /> received after nearly two years,you were given a deadline of May 20,2002 to submit the request. EHD has not <br /> received this request to date. <br /> Submit a written request to EHD by December 6,2002. The request should include information on usage of <br /> the wells,submission of annual report to EHD,estimated length of time you wish to keep the wells. Please be <br /> aware that you will becharged for review of reports and make site inspections as necessary according to E14D <br /> hourly rate fee(currently$89/hour). Enclosed is the billing form(Master File Record)required to be <br /> completed and returned to EHD prior to approval of your request to retain the monitoring wells. <br /> If the request is not submitted to EHD by the date directed;an Abatement Notice to destroy the wells will be <br /> issued. According to Section 6.1 of San Joaquin County EHD Well Standards:"When a well no longerfunctions as <br /> originally designed,or bas fallen into sueb a state of disuse... it must be destroyed under a well destruction permit." <br /> ,You were also notified that you are required to submit information to EHD regarding the current usage of the <br /> old domestic well located in the basement of the site. EHD has not received any information on the domestic <br /> well to date. <br /> Submit information regarding the domestic well to EHD by December 6,2002.
