The two monitoring wells(i W)on site were not destroyed. Ground Water
Monitoring Report dated June 1998 prepared by Advanced GeoEnvironmental,Inc.was the last monitoring
report submitted to EHD. Analytical results of the groundwater samples collected from the two MWs and one
domestic well in June 1998 were all non-detect for Total Petroleum Hydrocarbons as gasoline, aromatic
hydrocarbons (BTEC,and all five oxygenates.
In a meeting on April 17,2002,Central Valley Regional Water Quality Control Board agreed with San Joaquin
County Environmental Health Department(EHD)that the site should be considered for no action
status pending on the destruction of the two monitoring wells.
On December 10,1998,an"Application for Underground Storage Tank Closure Permit"was approved with
conditions by EHD. The condition required you to submit a written request to PHS/EHD if you wish to keep
the monitoring wells on site after no further action status of the above referenced site has been granted. EHD
ihas not received this written request.
Submit to EHD by May 20,2002 a written request to keep the MWs after closure of the site. The request
should include information on frequency of sampling and constituents to be analysed, and the estimated length
of time you wish to keep the monitoring wells. You are also required to submit any analytical results of
groundwater samples to EHD. If a request is not received,an abatement notice to destroy the MWslwillbe
issued.
1 An old domestic well was identified in the basement of the site. It is unknown if this well is currently in use.
Submit information of its current usage and frequency to EHD. If the well is not in use it must be destroyed in
accordance to Section 6.1 of San Joaquin County EHD Well Standards: "When a well no longerfunctionras ariginally
designed,or has fallen into sucb a state ofdisuse... it must be destroyed under a well destruction perntit," Submit information
on the domestic well usage to EHD by May 20,2002.
Please feel free to contact Jeffccy Wong at(209)468-0335.
Donna Heron,R Health
Director
En o ntal Health Division /
Jeffze yong,Senior REHS Ll Nuel Henderson,RG
LOP/Site Mitigation Unit IV LOP/ Site Mitigation Unit IV
c; CVRWQCB—Marry Hartzell
Ji
1
