SAN10 A Q U I N Environmental Health Department <br /> Complaint Record Daily Time and Activity Summary from 1/1/2019 to <br /> 7/26/2019 for COQDPHYHI located at <br /> 15250 N THORNTON RD, LODI, CA <br /> Activity Date Recorded by <br /> 06/05/2019 Received the following email updates from Richard Harness of LMH: PE 2546 FLORIDO <br /> 18 min SC 544 <br /> " I just received word from Patriot Environmental that they just received the EPA <br /> Generator ID No. today. They will be preparing manifests for transport of the 10 <br /> drums. I have asked them to send me copies of the initial manifests, which I can <br /> forward to you. They told me the waste will be transported to Crosby and Overton <br /> in Long Beach, and it could be as long as 2 weeks before we get final copies back <br /> from C&O. <br /> If at any time you are of the opinion that this project is not being handled in a <br /> timely manner, please call me immediately at the number below." <br /> and "Yesterday afternoon late I talked to the person makes all the disposal <br /> arrangements for Patriot at their Sacramento office. They have disposal lined up <br /> with Crosby & Overton at its Long Beach facility. In order to keep transportation <br /> costs as low as possible, they will be transporting the waste as part of what is <br /> commonly called a"milk run." They will pick up wastes from as many facilities as <br /> possible to try to get a full truck load. This is normally done about once a week, <br /> but sometimes more often when they have a lot of waste to pick up. The"milk <br /> run" concept sometimes slows the disposal process down by a few days, but it <br /> keeps the transportation costs down considerably. <br /> I'll keep you posted." <br /> I emailed Richard back asking him if the waste generated from this spill clean-up <br /> will have it's own uniform hazardous waste manifest. <br /> 06/07/2019 Received the following status update emails from Rich of LMH regarding the PE 2546 FLORIDO <br /> 18 min waste pick up: SC 544 <br /> Received on 6/6/19: "Yes. I'm not sure if there will be 1 manifest for all 10 drums <br /> or 10 manifests, 1 for each drum. I should have the preliminaries from Patriot <br /> Environmental soon." <br /> Received on 6/7/19: "Hi Elianna, I've attached a copy of the DTSC report with the <br /> EPA ID No. CAC003017936 for Pat Fruth Inc. regarding the subject waste. Patriot <br /> has informed me the waste will be picked up for disposal as soon as possible." <br /> Attached the received DTSC report to the complaint documentation. <br /> 06/24/2019 Reviewed a copy of the initial hazardous waste manifest, for the disposal of the 10 PE 2546 FLORIDO <br /> 18 min drums of waste, received from Richard Harness on 6/24/19. Richard states in his SC 544 <br /> email that he will provide the final manifest once he receives it. <br /> 07/10/2019 Reviewed a remediation report and the final manifest from Rich Harness of LMH PE 2546 FLORIDO <br /> 48 min received on 7/6/19. Emailed Rich back letting him know that if no further SC 544 <br /> information is needed I will submit his request for a No Further Action letter to my <br /> supervisor. Gathered complaint documentation and submitted to Garrett Backus, <br /> Lead Sr. RENS, for abatement. <br /> 07/12/2019 Responded to Rich Harness's request for a No Further Action Letter and attached PE 2546 FLORIDO <br /> 12 min my response to the complaint paperwork.
