PE 2546 FLORIDO <br /> 24 min SC 544 <br /> 05/28/2019 Called and spoke to Noel Liner(209-333-6878), Compliance Engineer for City of PE 2546 FLORIDO <br /> 18 min Lodi Surface Water Treatment Facility. Noel stated Flag City sewer lines are SC 544 <br /> connected to City of Lodi White Slough Water Treatment Facility, however, Flag <br /> City storm drain system is not connected to any City of Lodi lines. Noel <br /> recommended I reach out to SJC Public Works for more information about Flag <br /> City storm drain system. <br /> 05/29/2019 Called and requested Jesse Diaz, environmental contact for Love's Travel Stop, to PE 2546 FLORIDO <br /> 42 min provide verification or proof of where the impacted drain leads to. SC 007 <br /> Received a site map from Jesse Diaz. Reviewed the site map of Love's Travel <br /> Stop showing the impacted drains lead to a storm drain interceptor, then in the <br /> direction of the retention pond located north-west of the Flying J gas station west <br /> of Thorton Rd. Researched how storm water interceptor's work and discussed with <br /> Garrett Backus. <br /> 05/30/2019 Revised narrative by providing additional details. Submitted over-time earned slip PE 2546 FLORIDO <br /> 30 min and narrative to Garrett Backus for his review. SC 007 <br /> 05/31/2019 Added site map to Emergency Response Record form. Generated photo log. PE 2546 FLORIDO <br /> 36 min Emailed Rich and Ed of LMH Environmental requesting they provide a status SC 544 <br /> update of the disposal of the hazardous waste generated from this spill clean-up. <br /> Received the following response from Rich: "Hi Elianna, I was unable to get the <br /> info I needed from the trucking company so that Patriot Environmental could get a <br /> Temporary EPA ID No. to expedite disposal until today. I passed this info on to <br /> Patriot this morning. They have disposal arranged; it's just a matter of picking up <br /> the drums to transport them. This should be finalized by Monday." <br /> Replied back to Rich requesting he forward me a copy of the manifest once he <br /> receives it. <br /> 06/04/2019 Received an email from Lisa Palm of Pat Fruth Trucking PE 2546 FLORIDO <br /> 12 min (lisap@fruthtrucking.com; 320-5584209), asking if she had to do anything with the SC 007 <br /> notification of hazardous waste discharged she received in the mail. I responded <br /> letting her know the notification was for her own records, and notified Lisa that I <br /> am still waiting on an update regarding the disposal of the waste generated. <br /> Notified Lisa that if I do not receive the final manifest in a timely manner, I will be in <br /> contact with her or the owner of Pat Fruth Trucking. Notified Lisa all my time spent <br /> on abating this complain will be billed to the responsible party at$152/hour. <br /> Page 3 of 5 <br />
