The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact John Alaniz at (209) 468-3147 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. Verification of the dispenser footprints contained by the under dispenser containment <br /> sumps (UDCs) <br /> b. Visual inspection of the piping within the dispensers <br /> c. Verification of Bravo splash guard installation per the manufacturer's installation instructions <br /> 4. If concrete will be broken near the UDCs or piping, a secondary containment test must be <br /> performed on the impacted components once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal supervision <br /> of an individual physically present at the work site who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the Contractors <br /> State License Board (may be issued to the installation company), or a Tank Tester's license <br /> issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the International <br /> Code Council (ICC), and <br /> 1868 E.
