SANJ O A Q U I N Environmental Health Department <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> KIA COUNTRY 1 1515 N MAIN ST, MANTECA January 19, 2016 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> CCR 66262.42(a)(c)(d) Failed to comply with uniform hazardous waste manifest exception requirements. <br /> Manifest number 009436916JJK was found without a signed off copy from the destination facility. Hazardous waste <br /> generators shall retain copies of all manifests signed off by the destination facility on site for three years and have <br /> them readily available for review. If the generator did not receive a copy of the manifest with the handwritten <br /> signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the <br /> date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner <br /> operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of <br /> th signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, <br /> the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover <br /> letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. <br /> Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> J Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Submit to <br /> the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. <br /> This is a repeat violation, Class II. <br /> CR 66262.11 Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 years. <br /> No waste analysis was available for the oil waster separator waste. This waste is being disposed of as <br /> non-hazardous waste (last disposal occurred on 12-4-2015). This waste was historically hauled away on a <br /> hazardous waste manifest on 2-13-13 and 9-13-06. A generator shall make a hazardous waste determination and <br /> keep a record of any test results, waste analyses, or other determinations made in accordance with hazardous <br /> waste regulations for at least three years from the date that the waste was last disposed of. Immediately locate a <br /> copy of the waste analysis conducted for the oil waster separator waste and submit a copy to the EHD. If a copy is <br /> unavailable, immediately make a hazardous waste determination and begin managing the oil water separator waste <br /> under Title 22 hazardous waste regulations. <br /> This is a Class II violation. <br /> 109 CCR 66265.53 Failed to maintain a hazardous waste contingency plan onsite and submit to required agencies. <br /> The contingency plan was not available at the time of inspection. A copy of the contingency plan and all revisions to <br /> the plan shall be maintained at the facility at all times.
