Mar. 18. 2014 1 : 57PM No. 0767 P. 5 <br /> Encroachment Permit, Nu~inber irfir�l <br /> Special Conditions <br /> Page 2 of 2 <br /> elected officials, officers, agents, and employees) from and against any and all claims, <br /> demands, damages, liabilities, costs, and expenses resulting from or arising out of the <br /> performance of the construction work by the Permittee or its authorized representative(s) <br /> and/or their agents, representatives, its authorized representative(s) or employees. <br /> 7. The Permittee or its authorized representatives) shall obtain written permission(s) or <br /> agreement(s) for work that are to be performed by the Permittee or its authorized <br /> representative(s) outside City's right-of-way and/or easements, all at the Permittee or its <br /> authorized representative(s)'s cost and expense. <br /> 8. The Permittee shall also notify owner(s) and/or operator(s) of existing public utilities <br /> including AT&T (telephone and fiber optic facility), PG&E (gas and electric), Comcast <br /> (cable TV), and others, prior to the start of work around or adjacent their facilities, or <br /> work that may affect their facilities. The Permittee shall comply with all the utilities <br /> owner(s) and operator(s)'s requirements, all at the Permittee's sole cost and expense. <br /> 9. The Permittee shall install or apply 2 inches thick asphalt concrete overlay 25 feet from <br /> both sides of the utility trench or pavement excavation. In order to retain existing <br /> pavement grades or street cross slope, the Permittee will be required to grind the <br /> existing asphalt concrete pavement to a uniform depth of 2 inches over the entire area to <br /> be applied with asphalt concrete overlay. If the utility trench or pavement excavation <br /> encroaches on the next travel lane or crosses the centerline of a two-lane street or alley <br /> way, the extent of asphalt concrete overlay shall be the entire width of the adjacent <br /> travel lane or the entire width of the street or alley way for a two-lane street or the alley <br /> way. <br /> The pavement restoration work required under this sub-section must be completed b <br /> the Permittee within thi 30 calendar days from the date of issuance of this ermit <br /> unless changed in writing by the City Engineer. <br /> 10.
