i <br /> Levand-Bright Property Page 2 of 2 <br /> 3 East Eleventh Street January 21, 2014 <br /> Tracy, California <br /> For issuance of required permits, the following must be submitted to the EHD to the attention of <br /> Gabriel Gonzalez: <br /> • Three EHD Well and Boring Permit applications (one for each parcel/address); <br /> • Master File Record form; <br /> • Licensed Contractor's Declaration (LCD); <br /> • Encroachment permit; and <br /> • A permit/inspection fee of $1,170 ($390 x 2 + $130 x 3) to the EHD. The permit fee for <br /> borings per parcel is $130, along with a $390 Inspection Activity fee per parcel. <br /> This EHD approval is limited to the methods proposed in the Work Plan for boring advancement <br /> and abandonment, soil and groundwater sampling, equipment decontamination waste <br /> management, and technical justification for the proposed work.
