Date Corrective Action was Initiated as 08/31/00 (prior to your acquisition of site). <br />4. List All USTs at Subject Site. You only list one 500 gallon UST removed on <br />11/05/03. Were there ever any other USTs located at this site? <br />• Submit a map drawn to scale that shows all underground storage tank locations, a north arrow and <br />distances relative to the nearest public road. <br />• Claimant is requesting priority class "C" and must submit documentation verifying that claimant does <br />not employ more than 500 full time or part time employees. Submit documentation supporting the <br />number of employees (i.e. Department of Employment Development (DE6) for the last quarter or a <br />declaration letter signed by an officer of the company). <br />• Your Certification of Financial Responsibility is being returned for completion. It also needs to be <br />signed/dated by a Witness or Notary. <br />• Claimant must provide the following for the purchase of the site: <br />1. Purchase Offer <br />2. Purchase Agreement (final) <br />3. Appraisal Report <br />4. Escrow Instructions <br />• Claimants who acquire sites after January 1, 1990, must complete the enclosed Claimant Certification <br />of Compliance with Fund Regulations Section 2811(a)(1)-(2) and Health & Safety Code, Section <br />25299.54, subdivision (h). <br />AND <br />• A copy of the permit to own or operate the UST from the local implementing agency dated between <br />January 1, 1984 and January 1, 1990 (pursuant to Chapter 6.7 of the Health and Safety Code). <br />• Claimant has indicated that the USTs were removed post the December 22, 1998 deadline to have all <br />single walled USTs either removed or upgraded. Claimant must demonstrate that the following <br />actions were completed prior to December 22, 1998. <br />1. Date operation of USTs ceased. <br />2. Date USTs were emptied. <br />3. Claimant applied for a temporary/permanent closure permit for the USTs (Submit copyj. <br />Submit a copy of the latest permit to own or operate the UST from the local regulatory agency (Air <br />Pollution permits are not acceptable) prior to UST removal. <br />+ Submit a copy of the Underground Storage Tank Upgrade Certificate for the subject facility <br />identifying the site is in compliance with December 22, 1998 upgrade requirement. <br />• Submit a removal permit for all underground storage tanks listed in claim application. <br />• UST Removal Observation Report by the local regulator for UST removed on 11/05/03 (and for <br />previous USTs?). <br />California Environmental Protection Agency <br />�.r) Recyc%dPaper <br />
