(AGE) in response to the Fund's letter dated June 18, 2004. We find that the following additional information/clarification is still needed to determine your eligibility for placement on the Priority List: • On page two of your claim application, Under List All USTs at Subject Site, please list all USTs associated with this site (past/present-including the USTs supposedly removed in the 1970s). • I have revised page two of your claim application as indicated below; please clarify if incorrect: 1. Under Date Release Discovered you indicated 5110/00. I included 10/21/03 far the release discovery date for the 500 -gallon UST you owned. 2. Under Date Regulatory Agency Confirmed Release and Issued Cleanup Directives you indicated 6128100. 1 included 11/17/03 for the first directive to you for cleanup associated the 500 -gallon UST, 3. Under Date Corrective Action was Initiated you indicated 8131/00. I included 11/03 as the date you initiated corrective action for the 500 -gallon UST you owned. • The Claimant Certification of Compliance form is being returned for completion. Please complete the highlighted sections, with claimant's original re -signature and re -date. 1. When did you become aware of 500 -gallon USTs at this site? You answered "during excavation activities". Please include a date. 2. When and how did you first become aware of the requirement to obtain a permit for the subject UST? You answered "N/A". It appears that a removal permit was obtained on your behalf by A.G.E., with an expiration date of 1131/04 (there was a service contract between you and A.G.E. dated 10127103). 3. When did you first obtain a permit (either to own/operate or to remove) for the subject UST (same reference to number 2 above)? 4. Have you renewed your underground storage tank(s) permits, as required, from the date you first obtained a permit to the present time (or until the tank(s) removaI)? Answer Yes or No. If No, explain. . It appears that some corrective action work was done at this site for a release that was discovered on 5/10/00 and before claimant's acquisition on 2/15101.
