According to the designated operator monthly inspection reports and the facility alarm records, <br />brine was added to the under dispenser containment sumps (UDCs) in response to the following alarms: <br />10/25/13, 11/24/13, 11/25/13, 12/8/13, 1/2/14, 1/4/14. According to Mr. Bill Sodhi and Ms. Mayra <br />Reyes, the store manager, all brine inputs are done by Elite IV. Maintenance records for brine inputs <br />were not found on site. These records shall be kept on site for at least 3 years. Immediately locate and <br />maintain the maintenance records for all inputs of brine on site and submit copies to the EHD by May 3, <br />2014. <br />204. The designated operator failed to document the smart sensor (S) alarms from the attached alarm <br />histories on the designated operator monthly inspection reports and failed to check that they were <br />responded to appropriately. During the monthly inspection, the designated operator shall review the <br />alarm history for the previous month, check that each alarm was documented and responded to <br />appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. <br />305. The 87 fill sump annular sensor failed to activate an audible and visual alarm when tested to <br />ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to <br />activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. <br />The sensor was replaced and retested during the inspection. A "UST Retrofit Verification with Inspector <br />Already Onsite" form has been completed and provided to the operator and contractor. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector. <br />Received By: �{ <br />Title. <br />r �- <br />EHD 23-02 (Rev 0 1 9112 Page 3 UST INSPECTION REPORT <br />
