The facility failed to address all the alarms from the attached alarm history on the 5/15/17, 6/28/17, 7/24/17, 8/25/17, 9/7/17, 10/9/17, 11/16/17, 12/4/17, 1/23/18, 2/9/18, 3/5/18, and 4/3/18 designated operator (DO) monthly inspection reports and failed to check that they were responded to appropriately. The alarms that haven't been addressed include: 1) On the 5/15/17 DO report: L11 Disp 1/2 brine alarm on 4/24/17; L12 Disp 3/4 brine alarm on 4/27/17 and 5/5/17; L13 Disp 5/6 brine alarm on 4/18/17; L14 Disp 7/8 brine alarm on 4/18/17; L15 87 STP sump annular space alarm on 5/15/17. According to the 5/15/17 DO report, the report indicated that the "alarms on 5/15/17 were due to low brine, fluid has been removed and alarms were ok." This type of brine sensors do not have a high liquid alarm, and should not have fluids removed. Please provide clarification on actions taken on this DO report. 2) On the 6/28/17 DO report: L14 UDC 7/8 brine alarm on 6/28/17 indicates alarm is still on, and has not been addressed; L18 Vent annular space alarm on 6/19 indicates alarm is still on, and has not been addressed. 3) On the 7/24/17 DO report: L6 91 annular high liquid alarm on 7/23/17 was still on, and has not been addressed; L11 dispenser 1/2 brine alarm on 7/22/17 indicates it was still on, and hasn't been addressed; L18 vent annular space alarm on 6/19/17 indicates it was still on due to leaking brine, and has not been addressed. 4) On the 8/25/17 DO report: L11 dispenser 1/2 brine alarm since 7/25/17 and 8/18/17 indicates in alarm; L12 dispenser 3/4 brine alarm since 8/20/17; L14 dispenser 7/8 brine alarm since 8/23/17, L18 vent annular space alarm since 6/19/17. 5) On the 9/7/17 DO report: L6 91 annular space alarm on 9/4/17, 9/5/17, and 9/6/17; L11 dispenser 1/2 brine alarm since 8/18/17, L12 dispenser 3/4 brine alarm since 8/20/17; L14 dispenser 7/8 brine alarm since 8/28/17; L18 vent annular space alarm since 8/18/17. 6) On the 10/9/17 DO report: L14 dispenser 7/8 brine alarm since 9/28/17; L11 dispenser 1/2 brine alarm since 8/18/17; L12 dispenser 3/4 brine alarm since 8/20/17; L18 vent annular space alarm since 6/19/17. 7) On the 11/16/17 DO report: L11 dispenser 1/2 brine alarm since 8/18/17; L12 dispenser 3/4 brine alarm since 8/20/17; L14 dispenser 7/8 brine alarm since 9/28/17; L15 87 annular space alarm sine 11/13/17; L18 vent annular space alarm since 6/19/17. 8) On the 12/4/17 DO report: L11 dispenser 1/2 brine alarm since 8/18/17; L12 dispenser 3/4 alarm since 8/20/17; L14 dispenser 7/8 alarm since 9/28/17; L15 87 STP sump annular alarm since 11/3/17; L18 vent annular alarm since 6/19/17. 9) On the 1/23/18 DO report: L11 dispenser 1/2 brine alarm since 8/18/17; L12 dispenser 3/4 alarm since 8/20/17; L14 dispenser 7/8 alarm since 9/28/17; L15 87 STP sump annular alarm since 11/3/17; L18 vent annular alarm since 6/19/17. 10) On the 2/9/18 DO report: L11 dispenser 1/2 brine alarm since 8/18/17; L12 dispenser 3/4 alarm since 1/25/18; L14 dispenser 7/8 alarm since 9/28/17; L15 87 STP sump annular alarm since 2/5/18; L16 91 STP annular space on 1/30/18; L18 vent annular alarm since 1/25/18. 11) On the 3/5/18 DO report: L12 dispenser 3/4 brine alarm since 3/5/18; L14 dispenser 7/8 brine alarm on 2/23/18 - 2/27/18, and 3/5/18; 12) On the 4/3/18 DO report: L11 dispenser 1/2 brine alarm on 4/2/18; L12 dispenser 3/4 brine alarm on 3/5/18; L14 dispenser 7/8 brine alarm since 4/2/18; L18 vent annular space alarm since 2/26/18. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a Class II violation.
