<br />HSC 25290.1(d) Monitoring system not maintained to detect entry into secondary containment (after Jul 2004).
<br />The S4 Vents 91 STP sensor failed when tested (could not get high liquid alarm), and the L18 Vent annular senor
<br />failed when tested (could not go out of alarm). During the inspection, the service technician stated the vent
<br />electrical penetration was leaking brine, and no brine was observed in the reservoir, and brine was observed on the
<br />floor of the vent sump. In UDC 3/4, inadequate brine levels was found in the brine reservoir. The continuous
<br />monitoring system shall be maintained to detect the entry of the liquid or vapor phase of the hazardous substance
<br />stored in the primary containment into the secondary containment and to detect water intrusion into the secondary
<br />containment. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed
<br />component with a functional component (LG 113-x listed, if applicable) and obtain a permit within one business day
<br />from the EHD. If the failed component can't be replaced immediately, there is a possibility that the UST system may
<br />be red tagged to prevent fuel inputs.

<br />HSC 25290.1(e) VPH monitoring of the interstitial spaces of the UST system is not maintained.
<br />Designated operator inspection reports from May 2017 through April 2018 indicate alarms and leaks from the
<br />interstitial spaces of each UDC and sumps. According to those reports, brine has been added on 6/28/17, 7/24/17,
<br />8/25/17, 10/9/17, 11/16/17, 3/5/18, and 4/3/18. According to the work orders by Elite IV, brine was added on
<br />1/25/18, and on that work order, it states "sensor positions are leaking too much to clear." On the work order dated
<br />2/13/18, Elite IV stated "L18 Vent transition has a well known leak, and is pointless to keep refilling brine reservoir.
<br />Suggest site not waste brine." On the work order dated 6/20/17 by Elite IV, and L18 alarm was present, and
<br />according to the work order, "saw brine exiting from penetration boot." On the work order dated 7/12/17 by Elite IV,
<br />indicates vent sump, and UDC 1/2 both have low brine levels, and "1/2 UDC level would not stabilize, and Vent
<br />transition is leaking inside containment box." On the work order dated 6/13/17 by Elite IV, dispenser 1/2 brine levels
<br />were low, and they attempted to add 2 cups of brine. On the work order dated 5/31/17 by Elite IV indicates L18
<br />alarm vent sump due to low brine.
<br />The S4 Vents 91 STP sensor failed to indicate a high liquid alarm when tested. The interstitial space of the UST
<br />shall be maintained, as designed, under constant vacuum such that a breach in the primary or secondary
<br />containment is detected before the hazardous substance is released into the environment. Correct immediately by
<br />having a properly licensed, trained, and certified contractor replace the failed component with a functional
<br />component (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the failed
<br />component can't be replaced immediately, there is a possibility that the UST system may be red tagged to prevent
<br />fuel inputs.
<br />The L18 Vent annular sensor failed to indicate a low liquid alarm when tested. The sensors monitoring the liquid
<br />level in the interstitial space shall be maintained so that a breach in the primary or secondary containment is
<br />detected before the hazardous substance is released into the environment. Correct immediately by having a
<br />properly licensed, trained, and certified contractor replace the failed component with a functional component (LG
<br />113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the failed component can't
<br />be replaced immediately, there is a possibility that the UST system may be red tagged to prevent fuel inputs.
