PHS/EHD also <br /> requested fecal coliform testing and that groundwater investigation continue until boundaries <br /> defined. <br /> 1992 <br /> 03-31-92 PHS/EHD faxed communication to Wickland stating that quarterly data had not been <br /> submitted since June of 1991 and if Wickland failed to respond a warning letter would be <br /> issued. <br /> 05-11-92 PHS/EHD correspondence reviewing the Update Status Report #6,July 1991 -January 1992 <br /> and the Update Status Report #7, February 1992-March 1992. PHS/EHD requested <br /> additional monitoring wells to define the downgradient plume of contamination and that a <br /> work plan to perform the additional investigation requested be submitted by June 30, 1992. <br /> PHS/EHD also requested that quarterly reports be submitted in a timely manner. <br /> 07-23-92 PHS/EHD correspondence approving the Work Plan to Further Define the Lateral Extent <br /> of Groundwater contamination"dated July 6, 1992. PHS/EHD directed Wickland to proceed <br /> with efforts to obtain encroachment agreements from the property owners. <br /> 09-18-92 WEGE faxed permission packets to install two off site monitoring wells. <br /> 09-22-92 PHS/EHD issued permit # 92-3234 for the monitoring well (MW11) to be installed at 6426 <br /> Pacific Ave. PHS/EHD issued permit # 92-3235 for the monitoring well (MW12) to be <br /> installed at 6404 Pacific Ave. <br /> 09-24-92 Field work commenced for the installation of off site monitoring wells. Grouting was finally <br /> completed on September 28, 1992. <br />
