D Within 60 days of incident <br /> (40 CFR§112.4(a), 40 CFR§110.3, 40 CFR§304, 19 <br /> CCR§27031 <br /> 5.4.1 Verbal Notification Content <br /> When reporting a spill,be prepared to provide as much of the following information as possible: <br /> • Name, facility address, facility phone number <br /> • Type of material spilled <br /> • Source, cause, and estimated total quantity of the spill <br /> • Date, time, and duration of spill <br /> • Description of all affected media(e.g., water, land, air) <br /> • Damages or injuries caused <br /> • Known or anticipated health or environmental risks <br /> • Actions being used to mitigate the effects of the spill(evacuation, containment, etc.) <br /> • Other agencies that have been contacted <br /> 5.4.2 Written Report Content <br /> After verbal notification to various agencies and personnel, a written report must be issued <br /> containing pertinent details relating to the cause of the release, corrective actions taken, final <br /> results of corrective actions, and proposed plans to prevent future recurrence of the release. A <br /> follow-up written report(Section 304) must be filed with California's Office of Emergency <br /> Services (Cal OES) as soon as practical. <br /> In addition to the above, a written report must be submitted to the USEPA regional administrator <br /> whenever there is a discharge exceeding the following: <br />
