D Within 60 days of incident (40 CFR§112.4(a), 40 CFR§110.3, 40 CFR§304, 19 CCR§27031 5.4.1 Verbal Notification Content When reporting a spill,be prepared to provide as much of the following information as possible: • Name, facility address, facility phone number • Type of material spilled • Source, cause, and estimated total quantity of the spill • Date, time, and duration of spill • Description of all affected media(e.g., water, land, air) • Damages or injuries caused • Known or anticipated health or environmental risks • Actions being used to mitigate the effects of the spill(evacuation, containment, etc.) • Other agencies that have been contacted 5.4.2 Written Report Content After verbal notification to various agencies and personnel, a written report must be issued containing pertinent details relating to the cause of the release, corrective actions taken, final results of corrective actions, and proposed plans to prevent future recurrence of the release. A follow-up written report(Section 304) must be filed with California's Office of Emergency Services (Cal OES) as soon as practical. In addition to the above, a written report must be submitted to the USEPA regional administrator whenever there is a discharge exceeding the following:
