Health and Safety Plan <br /> 455 E lith St <br /> Tracy,California <br /> 3.3.10 Heavy Equipment Operation <br /> Physical hazards can arise from loading and off-loading heavy equipment from tractor-trailers <br /> and locating equipment to designated areas of use. Hazards will be mitigated by personnel <br /> avoiding close proximity to moving equipment and immovable objects. <br /> Belts and rotating parts have the ability to injure, crush, or amputate body parts and limbs. <br /> Personnel will avoid contact with any moving part.Any loose items will be removed before <br /> working around moving or rotating equipment.Any moving parts that can be guarded will have <br /> an appropriate machine guard installed. Equipment will not be operated without the proper <br /> guards in place. Repairs/adjustments will be done with the equipment stopped and locked out <br /> as appropriate. <br /> The following measures will be implemented for heavy equipment operation: <br /> • The minimum required work uniform for all field personnel (e.g. Level D protection) <br /> shall be general work clothes, steel-toed construction boots ANSI approved,safety <br /> goggles or glasses,work gloves, high visibility vest, hearing protection,and a hard hat <br /> (American National Standards Institute(ANSI) approved). <br /> • Adequate workspace shall be maintained during equipment operation. <br /> • Equipment shall be inspected for proper working condition prior to use. <br /> • Field personnel shall only approach operating equipment from the operator's angle of <br /> view, and only after making eye contact with the equipment operator. <br /> • A 10-foot perimeter will be maintained around all active equipment as a "Danger Zone". <br /> • Only trained and qualified persons shall operate individual equipment. <br /> 3.3.11 Traffic Hazards <br /> While heavy vehicular traffic is not anticipated at the Site. However,employees should always <br /> follow: <br /> • Park work vehicle between on-coming traffic and work zone to the extent feasible <br /> • Demarcate the work zone with high visibility traffic cones <br /> • Wear high visibility vest(minimum of Class II vest per Caltrans specification) <br /> • Follow all laws regarding pedestrian crossing of streets <br /> • Traffic considerations should be made to minimize driver frustration and confusion <br /> • Provide as much barrier between work area and traffic lanes as practicable <br /> • When possible, employees should not walk between the traffic cones and the active <br /> lane of traffic. <br /> • Equipment and supplies should be unloaded from the back of the vehicle or the <br /> sidewalk side of the vehicle to minimize the potential to travel in the active traffic lane <br /> Terraphase Engineering Inc.
