Health and Safety Plan <br /> 455 E lith St <br /> Tracy,California <br /> measuring devices will be utilized to detect any presence of harmful or dangerous atmospheres. <br /> Any floating or rotating machinery that are in the confined space are turned off and locked out <br /> prior to beginning any work. <br /> 3.3.7 Noise <br /> Appropriate hearing protection (ear muffs or ear plugs with a noise reduction rating of at least <br /> 20 decibels [dBA])will be used if individuals work near high-noise generating equipment (>85 <br /> dBA). Determination of the need for hearing protection will be made by the Project Leader. <br /> 3.3.8 Slips, Trips, and Falls <br /> Slips,trips, and falls are a leading cause of injury on construction and remediation worksites. <br /> Water and slime collect on horizontal surfaces and stairs, posing a potential hazard. Proper <br /> housekeeping is the key to preventing injuries of this nature.To minimize potential for injuries, <br /> the following measures will be implemented: <br /> • Site around open manholes, excavations,trenches, etc. will be kept clean and orderly <br /> • All potential trip and fall hazards will be clearly marked, modified to reduce the hazard, <br /> or engineered in a configuration to eliminate the hazard (if possible) <br /> • All floors and stair treads will be kept clean of water, oil, and polymer. <br /> • All hoses will be coiled and stored out of the way when not in use. <br /> • There will be no running on the site. <br /> As with all field work sites, caution will be exercised to prevent slips on rain slick surfaces, <br /> stepping on sharp objects, etc. Work will not be performed on elevated platforms without fall <br /> protection. <br /> Before conducting any work adjacent(such as opening, measuring water level, etc.)to an <br /> underground storage tank access area (e.g. manhole),the personnel will conduct a visual <br /> inspection of the area from the opening to identify any trip/slip hazards. Personnel, who are <br /> within 10 feet of the opening,will use full body harness and a self-retracting lifeline with fall <br /> arrestor.The fall protection system will be tied into a suitable anchor(e.g.tree,truck, etc.)that <br /> allows for work to be conducted around the underground storage tank opening.The access <br /> cover shall be completely closed and bolted to the frame (if bolts are present) prior to the fall <br /> protection system being removed. <br /> 3.3.9 Illumination <br /> Proper illumination is important in the prevention of slips,trips, and falls. It is anticipated that <br /> work will be conducted during daylight hours and not within any structures.Therefore, no <br /> additional illumination will be required. <br /> Appendices Terraphase Engineering Inc. <br />
