Health and Safety Plan <br /> 455 E 11th St <br /> Tracy,California <br /> 4.0 ROLES AND RESPONSIBILITIES <br /> The following section describes the roles and responsibilities of the personnel involved with the <br /> project. See Figure 3 (below)for a schematic of roles and levels of authority.Appendix A has a <br /> list of contact names and phone numbers related to this project. <br /> 4.1 Project Manager <br /> The project manager has the responsibilities to: <br /> • staff the project with qualified personnel (including proper training and certifications); <br /> • ensure proper documentation is being collected and maintained; <br /> • provide adequate resources and equipment to field personnel; <br /> • monitor the performance of field activities; and <br /> • ensure company policy is in compliance with all applicable regulations. <br /> 4.2 Project Leader <br /> The project leader will serve as the Site Safety and Health Officer(SSHO) and will report to the <br /> Project Manager. The project leader has the responsibilities to: <br /> • act as the liaison between field personnel and the subcontractor(s); <br /> • lead the daily and weekly safety meetings; <br /> • ensure the HASP is being followed; <br /> • provide adequate resources and equipment to field personnel; and <br /> • monitor the performance of field activities. <br /> 4.3 Subcontractor(s) <br /> Terraphase is responsible for the oversight and implementation of the project safety program. <br /> Each subcontractor will be required to designate one individual to work directly with and under <br /> the authority of the SSHO to ensure safety responsibilities are being met. Coordination and <br /> control of work flow will be achieved through frequent meetings (formal and informal) and job <br /> site inspection. The designated subcontractor safety designee (subcontractor project manager) <br /> must be granted stop work authority over persons from their company. <br /> Subcontractor control and coordination will be through the chain of command identified in <br /> Figure 2. Although each subcontractor is ultimately responsible for the performance and actions <br /> of their employees,the SSHO has the authority to take action when a violation of safety <br /> guidelines is in question. <br /> Appendices Terraphase Engineering Inc. <br />
