Additionally, an Excavation Permit from the SJC PHSIEHD or SJVUAPCD may also be necessary. Horizon or the selected excavation contractor will arrange for the acquisition of the permit(s) from the appropriate agency for the proposed site work. A-W 1 Destruction Permit will be acquired from the SJC PHS/EHD prior to the proposed water well destruction work, and if necessary, prior to the excavation of well MW-1. 6.4 Implementation Schedule It is Horizon's understanding that the CAP review process allows for public comments and vs input on the proposed Corrective Action. After completion of the 30-day review period, the SJC PHSXHD will contact the property owner, Ms. Michelle Nokes, with the results of the public review process. At that time, Horizon will address any appropriate questions or comments, then proceed with scheduling of the proposed Corrective Action work. The projected schedule for implementation of the proposed Corrective Action is during the summer of 1998. 6.5 Proposed Verification Monitoring In their letter dated May 13, 1998, the SJC PHSIEHD states that two quarters of groundwater sampling should be performed to verify the effectiveness of the remedial work.
