Upon its approval by the <br /> District this signed form may be used as proof (needed by the building official) of compliance with, or exemption from, the <br /> NESHAP notification requirements. L <br /> Submit this form to the building department with your apptication for a demolition permit. <br /> , <br /> Applicability <br /> Facilities subject to the NESHAP (regulated facilities) include all commercial buildings, apartments with more than 4 units, other f <br /> structures and non-portable equipment.
