All conductors on the construction site that will not be removed by <br /> V excavation must be plugged with cement and leak tested by a Division <br /> L inspector. Any conductor must be plotted on the construction site plan and <br /> listed on the Construction Site Plan Review. A recommendation for a vent <br /> system should be made when appropriate, for consideration by the local <br /> L permitting agency. <br /> J. If an applicant does not comply with the Division's reabandonment statute <br /> L requirements, which may be modified at local jurisdiction direction, the <br /> District should notify the local permitting agency of the situation. If <br /> warranted, a letter of concern should be written to the local permitting <br /> 6. <br /> / K. The outermost casing stub of all wells within the construction site should be <br /> maintained within 5 to 10 feet below the surface of final grade. The casing <br /> ` can be cut off ora riser s r added to achieve the required depth. The District <br /> Deputy may grant variances for proposed utilities, channel construction, <br /> i special vent systems, swimming pools, etc. <br /> L. Foll- r(5-ocation verification and testing, the applicant must correctly <br /> plot the well location on the construction site plan, which will be supplied to <br /> the Division for the Construction Site Plan Review. The Division will keep a <br /> copy of any well location surveys in the project folder and the well file, and <br /> enter the data in an appropriate database. Each well must be identified with <br /> the official name of the company/operator and well designation. Wells in <br /> proximity, but outside the construction site, should also be plotted to verify <br /> their location. <br /> The applicant must submit at least three copies of the construction site plan <br /> to the local permitting agency or district office to satisfy requirements. of the <br /> Division's Construction Site Plan Review program. The construction site <br /> plan should indicate the property and/or construction site boundaries, <br /> footprints of proposed structures and existing structures that will remain <br /> existing and proposed oilfield facilities (tanks, processing equipment, <br /> enclosures, pipelines, etc.) that will be operating on the site after <br /> completion of the proposed development. Also, the applicant should include <br /> ` a description of the existing type and height of all facility enclosures (e.g., <br /> fencing) and list any modifications that are part of the construction site plan. <br /> All well work must be completed prior to any construction site plan approval <br /> being issued. <br /> M0118116 February 2004 <br />
