A Division engineer will review the submitted plans and prepare a <br /> Construction Site Plan Review on a modified Form OG190 (see Exhibit <br /> ow 181.2, Item 5), which may indicate requirements and recommendations <br /> regarding surface mitigation for natural gas, any active wells and <br /> facilities that will remain after the development is completed, and <br /> r visibly contaminated soil_ There is a list of standard, acceptable <br /> comments that can be used (see Exhibit 181.2, Item 6). In addition to <br /> Form OG190, a cover letter should be prepared (see Exhibit 181.2, <br /> Item 7). <br /> A Division reference number must be assigned to each construction <br /> site plan. The number may consist of the district's number, two or four <br /> letters (two for a city and four for a county), two numbers <br /> (corresponding to the year of certification or review), and three <br /> numbers (corresponding to the project number). When the review process is complete, the reviewing engineer may affix <br /> a Division certification or review stamp to all three copies of the <br /> construction site plan and all three copies of the Plan Review (Form <br /> OG190). <br /> One copy of each should be sent to the local permitting agency and <br /> the applicant, with the third copy retained at the district office. <br /> The following disclaimer shall be affixed to all plans stamped with a <br /> certification stamp: <br /> "The use of the word 'certify' or 'certification' constitutes an expression <br /> of professional opinion based upon information submitted by the <br /> developer/landowner, with respect to the well and environmental <br /> review, and does not constitute a warranty or guarantee, either <br /> expressed or implied." <br /> M01181/7 February 2004 <br />
