IV. POTENTIAL CAUSES OF SPILLS <br /> Equipment Failure <br /> — Pneumatic equipment may fail physically, such that oil is <br /> lost. Such failures are minimized by monthly inspections and <br /> maintenance. <br /> Human Error <br /> This is always a possible cause of spills. Human error is <br /> minimized by having sufficient numbers of properly trained and <br /> supervised personnel on site. Safety and equipment reliability <br /> are stressed in routine safety meetings. Spill prevention and <br /> control are discussed in required annual refresher training <br /> programs. <br /> — Vandalism and Sabotage <br /> Vandalism has not been a problem on-site. Acts of vandalism <br /> _ or sabotage could cause spills, but it is unlikely that such acts <br /> would cause discharges beyond the constructed containment berms <br /> and water control facilities, or off the site. <br /> — Field Operations <br /> The most likely spills in the field result from two <br /> operations: <br /> 1 ) Diesel fuel servicing: Fueling the equipment may <br /> occasionally cause losses of up to two gallons. <br /> 2) Equipment: Spills ranging from one to 50 gallons may <br /> result from a hose break. <br /> — 5 <br />
