Broadway Avenue <br /> Conditions for 1905 N. Broadway Avenue: <br /> The Ordinance Requirements listed below have been identified as <br /> pertinent to this project. Other Ordinance Requirements may also <br /> apply. Ordinance Requirements are not appealable. <br /> A <br /> 1. Submit application for a hazardous waste permit to the <br /> Environmental Health Division. <br /> 2 . If floor drains are required, they must be plumbed through a <br /> sand/oil separator and discharged to an above ground system <br /> approved by the Environmental Health Division. This system <br /> must be designed by a registered engineer. Applicant must <br /> file an application for facility permit/waste discharge with <br /> the California Valley Region. The application forms can be <br /> obtained from the Environmental Health Division. If the above <br /> ground holding facility had to be emptied you first would have <br /> to analyze the contents for any hazardous material . You can <br /> obtain specific test requirements from the Environmental <br /> Health Division. + <br /> 3 . If hazardous material is stored on site applicant must <br /> register with County Office of Emergency Services. <br /> 4 . If hazardous waste is generated applicant must obtain a EPA <br /> identification number and comply with applicable regulations. <br /> 5. State on revised plans or improvement plans the maximum number <br /> of employees the sewage disposal system is being designed for <br /> and that the leach field area will be barricaded so it cannot <br /> be driven over, parked on or used as a storage area. This <br /> leach field area must be used for that specific purpose only <br /> and cannot contain any underground utility lines. <br /> 6. Construct an individual sewage disp <br /> the time of development. osal system(s) under a <br /> permit and inspection by the Environmental Health Division at <br /> A Division of San Joaquin County Health Care Services <br /> iM <br />
