n <br /> November 30, 2006 <br /> NOA Project Number: E06146A <br /> F13.L9 Water Supply and Sewage Disposal System <br /> The EHD well and septic permit files were searched on November 6,2006 in an effort to obtain copies <br /> F1of permits for any water wells or septic tanks on the subject property. Four pertnits were found. <br /> 1953 - 1020 South Carroll Avenue -Additional leach drain to existing one. <br /> 1961 - 1019 South Dawes Avenue- Install new septic system for trailer. <br /> 1965 - 1020 South Carroll Avenue -Additional Drainage, disposal field. <br /> 1987 - 1020 South Carroll Avenue - Destruction of septic system. <br /> It appears that the septic system located at 1020 has been removed,however it's possible that the septic <br /> system at 1019 still exists. No permits were found for a well, however on the 1953 permit a well is <br /> shown north of the residence. <br /> ! <br /> IM Department of Oil, Gas, and Geothermal Resources <br /> i <br /> �I The California Department of Oil, Gas, and Geothermal Resources (DOGGR) well map 60610 was <br /> reviewed for evidence of oil or gas wells on or in the vicinity of the subject property. No wells are <br /> depicted within one-quarter mile of the subject property. <br /> 3.1.7 Site Reconnaissance <br /> RA visual reconnaissance of the subject property was conducted on November 21, 2006. Photographs <br /> of the subject property are attached to this report as Appendix B. A site visit/assessment checklist is <br /> i attached to this report as Appendix D. <br /> 5 <br /> The subject property was observed to be a rectangular parcel. On the west half of the property was a <br /> single-family residence was under construction. A detached garage,two storage sheds,and bungalow <br /> were also located on the property. Two mail boxes were located for the subject property, 1018 and <br /> 1020 S. Carroll Avenue. <br /> A pile of concrete debris connected with the construction of the residence was observed on the drive <br /> way of the subject property. <br /> 1 <br /> The eastern half of the property consists of a storage structure,sidewalk,and field of grass.The ground <br /> was uneven and a mound and hole were observed. A pipe was located in the ground near this mound <br /> and hole;this is possibly the location of the septic system,which may or may not still be there. <br /> Surrounding properties were surveyed at the time of the site reconnaissance. To the north and south <br /> of the subject property were residences. To the west was Carroll Avenue,then residences.
