Two monitoring wells to be installed to first water for lateral definition of the groundwater contaminant plume. <br /> a <br /> 2. One deep,discretely screened monitoring well to be installed to 100'bsg near the former dispenser island area. <br /> The well will be drilled with a mud rotary drill ng and built with a pre-packed 5-foot long well screen. <br /> 3. Soil samples for analysis will be collected from each boring. In the deep boring,sampling will begin at 75-feet bsg. <br /> COMMENTS AND CONDITIONS: <br /> 1. Submit to PHS/EHD a well permit application and required$89.00 permit fee. <br /> 2. Fott},eight hours notice to PHS/EHD is required prior to initiating fieldwork . <br /> 3. Quarterly groundwater monitoring of all on-site wells will continue until du,ected otherwise by PHS/EHD. <br /> i <br /> 4.
