3. PUBLIC HEALTH SERVICES: <br /> a. No excavation is permitted into the groundwater table without specific approval from the <br /> 'Regional Water Quality Control Board, Central Valley Region. <br /> b. The materials from an excavation shall not be deposited into any water course nor in any <br /> way contribute to the pollution thereof. <br /> R <br /> C. The quarry shall not cause health or sanitary hazards and shall not create conditions <br /> which will cause the breeding or increase of mosquitos, rodents, or other pests. <br /> (Development Title Section 9-1415.2{i}) <br /> d. Provide an approved potable water supply for all employees, (Development Title Section <br /> 9-1415.2{i}} <br /> e. Provide approved toilets for all employees (chemical toilets are acceptable). <br /> (Development Title.Section 9-1415.2{i}) <br /> f. Provide handwashing facilities on or near the approved toilets. (Development Title <br /> Section 9-1415.2{i}) <br /> g. Septic system leach fields shall be prohibited in the pit bottoms. <br /> h. As a basis for initial and ongoing monitoring of water quality and groundwater levels in <br /> the vicinity of the quarry, the applicant shall annually submit to the Environmental Health <br /> Division a report on the status of water supply wells around the general perimeter of the <br /> permitted mining area. A copy of the annual report shall be submitted to the <br /> Development Services Division and affected property owners. The number and location <br /> of monitoring wells shall be determined by the Environmental Health Division, provided <br /> there shall be a minimum of ten monitoring wells for groundwater levels and five <br /> monitoring wells for water quality (two shall be up-gradient from the project site,two shall <br /> be down-gradient from the project site, and one shall be on the site). <br /> In the event the applicant is unable to obtain or retain the required water wells for water <br /> quality testing,the applicant shall procure new wells for monitoring purposes at locations <br /> acceptable to the Ermvironmental Health Division. The wells to be tested for groundwater <br /> quality shall, at a minimum, be tested for general minerals and nitrates. In addition, j <br /> tested wells that are used for domestic purposes shall also be tested for bacteria The <br /> applicant shall pay a fee to .the Environmental Health Division to cover the cost of <br /> reviewing the annual water quality reports. The fee shall be based upon the <br /> Environmental Health Division's hourly billing rate. <br /> In regard to the on-site well, should the number of nonresident individuals exceed 24 for <br /> at least 60 days per year, or the number of service connections exceed four, a yearly <br /> permit to operate a public water system under the provisions of Title 22, CCR, will be <br /> required by the Environmental Health Division. <br /> Conditions of Approval - 11 - QX-91-5 <br />
