WELL DESTRUCTION INSPECTION—PERMIT#SR0066665 <br /> On February 22,2013 a well destruction inspection was conducted at 27100 E. Flood <br /> Road. Specifics of the well destruction were discussed with Sergeant Chanda Basset of <br /> the Sheriff s Office who stated that the actual backfilling of the well with cement had <br /> taken place on 2/20/2013. The bottom portion of the well had been grouted with cement <br /> with the remainder of the well shaft backfilled with soil. During this process the wooden <br /> support structure had been left in place and the CMP had been removed. <br /> At the time of this inspection it was observed that the CMP had been removed and the <br /> well had been sealed to grade (refer to attached photographs). It was also observed that <br /> heavy equipment was in the process of grading and compacting the soil above and around <br /> the destroyed well. <br /> 'Senior HS <br />
